About the Practice
Van Eerd General Practice is located within Bredero Health Centre in Breda. We provide personalized care, focusing not only on illness but also on the individual and their resilience. You will be working in a professional and welcoming team consisting of GPs, a Physician Assistant, Medical Assistants, a Mental Health Practice Nurse (POH-GGZ), and a Practice Manager. We also work closely with the other GP practice within the health centre, Bredero GP Group.

What will you do?
As a Practice Nurse (POH-S), you will be the primary point of contact for patients with chronic conditions. Your responsibilities will include:

• Independently running clinics for patients with Diabetes Mellitus, Asthma/COPD, and Cardiovascular Risk Management (CVRM).
• Providing support within elderly care and smoking cessation programmes.
• Motivating patients to take an active role in managing their own health, in line with the Positive Health approach.
• Working closely with GPs and external healthcare professionals, including physiotherapists and dietitians within the health centre.

What do you bring?

• A completed qualification as a Practice Nurse in Somatic Care (Bachelor's degree level or equivalent).
• A patient-focused and proactive approach to work.
• Preferably experience working within a general practice setting.

What do we offer?

• A contract for 28 hours per week, with the possibility of increasing hours by mutual agreement.
• Salary and employment conditions in accordance with the Dutch GP Care Collective Labour Agreement (CAO Huisartsenzorg), including:
• 8% holiday allowance.
• End-of-year bonus.
